Who May Use the Web Site
The Web Site is not intended or designed to attract users under the age of 18. We do not collect Personal Information from any person we know to be under the age of 18. If you are under 18, we ask that you not send us any Personal Information.
Collection of Personal Information
The Web Site collects Personal Information from you when you provide it to us. The types of Personal Information we may collect include, without limitation, first and last name, age, street address, city and state, email address, telephone number, gender, birth date, credit card account information, and any other information that may identify you. Registration is an optional service provided by our Web Site. You do not have to register to browse our Web Site, but you do have to register to purchase products on our Web Site. When you purchase PhysioTru Products from our Web Site, you must submit payment information, including without limitation your credit or debit card number and expiration date, as well as your zip code. Such information may be recorded by PhysioTru to process your orders. In addition, when you return previously purchased products or request a refund or exchange, you may be asked to give your name, address, driver’s license number, and other identifiable information. PhysioTru collects this information to help prevent fraud and identity theft. If you contact any employee of PhysioTru or any affiliate of PhysioTru, a record of that correspondence may be kept.
The Web Site allows users to make Personal Information available in public forums such as bulletin boards, to the extent that such users choose to disclose such information. How to Exercise Your Rights to Access and Deletion:

For us to honor your access or deletion request, you must provide us with enough information to reasonably verify you are the person about whom we collected personal information or an authorized representative. We may ask you for information associated with your account, which might include your name, email, address, and phone number. We will confirm receipt of your request within 10 days and respond to requests for access and deletion within 45 days. We will also let you know if we are not able to verify your identify based on the information you have provided. If you submit a request for deletion, we will send you a confirmation email to validate the request before deleting your personal information.
After we have responded to two requests from you within a 12-month period, we may choose not to respond to any additional requests from you during such period.
You may designate an authorized agent to make a request on your behalf. If you choose to use an authorized agent, we may require that you both provide your authorized agent with written permission to make a request on your behalf and verify your identity directly with us. Section 1798.83 of the California Civil Code permits California residents who have provided their personal information to us to request data regarding our disclosure of their information to third parties for direct marketing purposes.
